Universal Plan Requirements
All NJ Medicaid managed care plans require documentation of:
- Prior Authorization (PA) submission.
- Established Medical Necessity.
- Documented Step Therapy completion.
- Diagnosis of Moderate-to-Severe Psoriasis.
NJ MEDICAID PLANS
The 4-Stage Step Therapy Pipeline
To clear insurance hurdles, document trial and failure (or contraindication) through these stages:
1. Topical Therapies (Required | >= 4 Weeks)
- High-potency topical corticosteroid (e.g., clobetasol) PLUS a Vitamin D analog (e.g., calcipotriene).
- Document inadequate response, intolerance, or disease too extensive for topicals.
2. Phototherapy (Often Required | 8–12 Weeks)
- NB-UVB preferred.
PDF - May be waived if: Logistically unfeasible (work/transport), severe disease, or special areas are involved (face, scalp, genitals, hands/feet).
3. Systemic Therapy (Required | >= 12 Weeks)
- Trial of at least ONE: Methotrexate (preferred by plans), Cyclosporine, or Acitretin.
4. Biologic Eligibility (ILUMYA Stage)
- Patient must meet at least ONE criteria: BSA >= 10%, special areas involved, severe disease, or documented severe impact on quality of life.
